Below are multiple ways that you can register and pay for the class of your choice.

Mail / Fax / Email

By using the below Registration and Credit Card Sheets you can print the forms and mail them to us at:

You can also print, complete, and then scan and email the sheets to us at:

  • Registration Sheet
    This registration sheet is used for all Priority Health Education classes. Select "registration sheet" above, print and send to us to put a temporary hold on a seat in the class of your choice.
  • Credit Card Sheet
    This credit card sheet is used to pay your 20% deposit or your full tuition for the class of your choice. Once at least a 20% deposit is paid, along with a completed registration sheet, your seat will be "reserved."
  • Unless other arrangement are made full payment must be received no later than 10 days prior to the start of the class to maintain your reservation.
  • You can use Visa, MasterCard, Discover or Amex.

Pay Pal

If you wish to pay via Pay Pal you can email us and request we send you a Pay Pal invoice. Be sure to specify the class, the class dates, and the class location.

Checks will not be accepted less than 10 days before the class starts without prior arrangements being made.
